In keeping with the Centers for Disease Control and Prevention guidelines for essential employee monitoring, UW-Whitewater is implementing a self-check survey to be completed by every employee who reports to campus in person for work or other needs. The survey is to be completed each workday before reporting to any campus facility. This survey is not required for those employees who are working remotely and are remaining off campus.
If you are not able to access the online survey, you can call the COVID-19 hotline at 262-472-1362 between the hours of 7 a.m. and 3 p.m. Monday-Friday to have the survey filled out verbally.
