Vaccination cards now approved as COVID-19 vaccination verification
Vaccination cards are now an approved form of verifying COVID-19 vaccination. (Previously they were not.)
If you previously submitted your vaccination card as verification but were denied, please resubmit your card.
If you are fully vaccinated — meaning you are at least two weeks past your last immunization date — and wish to receive an exemption from COVID-19 testing, you can submit your immunization record (including vaccine cards) in any of the following ways:
- Submit it online at https://my.uww.edu/Vaccination/
- Bring the copy to the COVID-19 office, located in the UW-Whitewater Police Department at Goodhue Hall
- Fax it to 262-472-5746
- Mail it to UW-Whitewater Police Department, 790 W. Starin Rd. Whitewater, Wisconsin, 53190
